Lost or Damaged Citizenship Documents?

Naturalized Citizen? Use Form N-565 to Replace:

  • Certificate of Naturalization

  • Certificate of Citizenship

  • Declaration of Intention (if lost, stolen, damaged, or your name changed)

Born a US Citizen? This Form Isn't For You:

If you've never been naturalized, use these documents instead:

  • US Passport

  • Certified Birth Certificate (issued by a US state/territory)

  • Consular Report of Birth Abroad (CRBA - born abroad to US citizen parents)

  • Certificate of Citizenship (if applicable)

Replacing a Citizenship Document (Issued Certificate)?

Form N-565 might still be an option for replacements due to loss, theft, damage, or a name change. Double-check the form's instructions to confirm eligibility before applying.
