Replacing Your U.S. Citizenship

Need a replacement U.S. citizenship document (certificate, declaration, etc.)? Here's what you'll need to do:

  • Complete Form N-565: Fill it out accurately with your details, immigration history, and the reason for needing a replacement.

  • Pay the Filing Fee: Check the USCIS website or form instructions for the current fee.

  • Proof of Citizenship:

    • Include a copy of your old document (if available).

    • Provide your U.S. passport (or other proof) if applicable.

    • Show documents like birth certificates if needed.

  • Explain Why You Need a Replacement:
    Lost, stolen, damaged, or has errors? Briefly explain the situation.

  • Two Passport Photos: Recent (within 6 months) and meeting USCIS requirements (check the form instructions).

  • Extra Documents (if requested): USCIS might ask for additional documents depending on your situation. Follow their instructions.

  • Optional E-Notifications (Form G-1145): Get email or text updates on your application by including this form.
