Lost or Stolen Child’s Citizenship Certificate?

Your child’s Certificate of Citizenship (Section 322) is lost or stolen? Don’t worry, you can get a replacement with Form N-600K!

Steps to Take:

  1. Gather Documents:

    • Find a copy of the original certificate (if available).

    • Collect documents proving your child’s identity, citizenship, and eligibility (birth certificate, passport, adoption decree, etc.).

  2. Complete Form N-600K:

    • Download the form from USCIS website and fill it out carefully.

    • Indicate the lost/stolen certificate on the form with any details you remember.

  3. Prepare Supporting Documents:

    • Include copies of birth certificate, passport, adoption decree (if applicable), and proof of citizenship.

    • If the original certificate is lost, provide additional evidence like witness affidavits.

  4. Submit the Application:

    • Combine Form N-600K with supporting documents and pay the filing fee.

    • Mail the package to the designated USCIS Lockbox facility.

  5. Follow Up:

    • USCIS will process the application and might schedule biometrics (fingerprints) for your child.

    • Track the status online or contact USCIS for updates.

    • Upon approval, a replacement Certificate of Citizenship will be mailed to you.
