Steps to Take:
Gather Documents:
Find a copy of the original certificate (if available).
Collect documents proving your child’s identity, citizenship, and eligibility (birth certificate, passport, adoption decree, etc.).
Complete Form N-600K:
Download the form from USCIS website and fill it out carefully.
Indicate the lost/stolen certificate on the form with any details you remember.
Prepare Supporting Documents:
Include copies of birth certificate, passport, adoption decree (if applicable), and proof of citizenship.
If the original certificate is lost, provide additional evidence like witness affidavits.
Submit the Application:
Combine Form N-600K with supporting documents and pay the filing fee.
Mail the package to the designated USCIS Lockbox facility.
Follow Up:
