How to obtain a copy of your Wage & Income Transcript?

Online Request:                                                                                                                                    

• Go to www.irs.gov/transcript

  1. Choose to "Get Transcript ONLINE" (immediately), OR "Get Transcript by MAIL" (5 to 10 days)                                    

  2. Follow steps on the IRS website to complete the request

  3. Select Wage & Income Transcript for the appropriate year

Paper Request:                                                                                                                                

• Form 4506-T, to access form click on 4506-T 

  1. Be sure to select OPTION 8 on the 4506-T form

  2. You can MAIL or FAX form. Instructions on 2nd page of 4506-T form
